Health Insurance Single coverage is provided to the employee at $84.00 co-pay to the employee. (Family coverage is available at an additional cost to the employee). HealthSmart Benefit Solutions administers this program. Inc. Note: The Summary Plan Document and links to other benefit sites are available through www.gbabenefits.com.

Dental Insurance Single coverage is provided the employees participating in the health insurance plan. (Family is available at an additional cost to the employee). Delta Dental administers this program.

Vision Insurance The vision coverage is provided to the employee, employee'e spouse, and family. The employee pays for the coverage. Uncicare administers this program

Group Life Insurance of $8,000 to 15,000.00 is provided to the employee. Unum administers this program.

Voluntary Term Life Additional coverage is provided to the employee, employee's spouse, and family. The employee pays for this coverage. This coverage is through Unum.

Pension Coverage is provided to the employee. The employee currently accrues 1.25 percent of their salary for every year of service. Vesting in the plan is ten years. All employees working 900 hours during a fiscal year are required by the IRS regulations to be covered. This plan is not optional. This program is administered through the Office for Finance and Insurance. Mercer Human Resources Consulting is the plan actuary.

Section 125 or cafeteria plan This benefit is available to all employees where they can have their share of insurance premiums deducted pre-taxed. This program is administered through AFLAC.

Tax Shelter or Deferred Annuities or 403(b) plan This is available to all employees. The employee provides for this coverage through a salary reduction agreement. Currently Standard administers and assists in the IRS compliance of this program and is the only vendor allowed in the diocese for 403(b) plans. (Diocesan priests may participate).

Social Security Benefits All employees contribute to social security through payroll deduction, which is matched by the employer. (This does not pertain to diocesan priests who are considered self-employed for Social Security Benefits, religious are exempt from social security).

Unemployment Insurance The Diocese of Springfield in Illinois is exempt from unemployment insurance. However, this benefit is offered to parishes with schools.

Workers' Compensation Insurance The Diocese of Springfield in Illinois provides Workers' Compensation to all employees as required by the State of Illinois Statues.
